Power of Attorney Form

If you have not completed a Power of Attorney (POA) form since January 1 of this year, please complete our updated POA form and have it notarized. This updated form allows us to handle all aspects of trailer registrations on your behalf.

If you are completing your trailer registration online, please upload the completed POA form as part of your submission. If you prefer to mail in your registration, simply print and include the form with your paperwork.
